COVID-19 VACCINATION RECORD CARD
The COVID-19 Vaccination Record Card, issued by the U.S. Centers for Disease Control and Prevention (CDC), is the official vaccination certification. The Record Card verifies the manufacturer, lot number, date and site of vaccination. Currently, three vaccines are authorized in the United States to prevent COVID-19:
· Johnson & Johnson / Janssen
These three vaccines are also recognized by the European Medicines Agency. Additional information about the CDC COVID-19 Vaccination Record Card can be found at https://www.cdc.gov/coronavirus/2019-ncov/vaccines/expect.html
