COVID-19 VACCINATION RECORD CARD
The COVID-19 Vaccination Record Card, issued by the U.S. Centers for Disease Control and Prevention (CDC), is the official vaccination certification. The Record Card verifies the manufacturer, lot number, date and site of vaccination. Currently, three vaccines are authorized in the United States to prevent COVID-19:
· Pfizer-BioNTech
· Moderna
· Johnson & Johnson / Janssen

CLARIFYING NOTE TO THE PREVIOUS ORDER FROM THE HEALTH AND FAMILY DEPARTMENT FROM 16 DECEMBER 2021
If you encounter any issues with requirements for an EU COVID passport by a business or organization that does not recognize the CDC card, you can present the Clarifying note to the order from the health and family department from 16 December 2021.
